Overview

For this Performance Task Assessment, you will select a population health program/policy evaluation that has been completed and assess the outcomes of the population health program/policy evaluation.

Your response to this Assessment should:

Reflect the criteria provided in the Rubric.

Adhere to the required assignment length.

• This Assessment requires submission of one (1) files, a completed Population Health Program/Policy Evaluation Analysis. Save the file as PA005_analysis_firstinitial_lastname (for example, PA005_analysis_J_Smith).

Achievement vs. Mastery of the Competency

• Mastery of this Competency means that 80% or more of the rows have been assessed as Exceeds Expectations and no rows have been assessed as Does Not Meet Expectations.

Achievement of this Competency means that all rows are assessed at Meets Expectations or above (but lower than the 80% threshold required for Mastery).

Assessment of Individual Rubric Rows

In order to achieve Meets Expectations for a particular row of the rubric, you must have adequately completed all criteria in that row. This means that you have addressed all required elements to the required level of quantity and/or quality.

In order to achieve Exceeds Expectations for a particular row of the rubric, you must have adequately completed all criteria in the row, and in addition, your response must reflect a depth and breadth of knowledge and expertise. Examples of this include—but are not limited to—the following:

• Research and select a population health program/policy evaluation report that has been completed and shared. Based on the population health program/policy evaluation you selected, address the following in the Population Health Program/Policy Analysis Template:

Explain which stakeholders were approached or involved in conducting the population health program/policy evaluation. Which stakeholder would benefit most from the results and reporting of the program/policy evaluation that was conducted? Be specific and provide examples.

Explain whether the population health program/policy successfully met the original intent of the outcomes that were identified. Was the population program/policy successful? Why or why not?

Explain whether you would recommend continuing or implementing this population health program/policy. Be specific and provide examples.
