NUR 315 Milestone Two Nursing Care Plan Guidelines and Rubric

In Milestone One of the final project, you will have chosen a well-known individual from history or current popular literature, with instructor approval, who has a known physiological—as opposed to psychiatric or psychological—disease or illness. In Milestone Two, you will analyze the disease or illness using a systematic approach. You will also develop a plan of care for that disease or illness.

Your plan of care should utilize the PIE (planning, intervention, and evaluation) format. The planning (P) section should include at least two short-term and two long-term goals that are most appropriate for your chosen case, and each goal should have measurable criteria and have a hypothetical target date or time. The intervention (I) section should include interventions or nursing actions that directly relate to the selected case’s goals and that are specific in action and frequency. The number of interventions should be appropriate for helping to meet individual goals. Finally, the evaluation (E) section of the plan of care should include proposed measures for determining success of the plan and an evaluation of the potential success of the plan based on those measures. You may write the plan of care in either narrative format or as a table with appropriate in-text citation used to support the plan of care.

Specifically for Milestone Two, the following critical elements must be addressed:

Planning for care for your selected case

Write a goal statement that is patient-centered and contains at least one measurable criterion or target date/time. In order to address this element comprehensively, you should be sure to provide at least two short-term and two long-term goals for your selected case.

Analyze the critical nursing concerns associated with this disease state and prioritize them from a treatment standpoint.

1. Identify patient care technologies that are appropriate for managing clinical manifestations of your selected case.

Intervention

Outline appropriate nursing interventions for your selected case. Be sure to substantiate your claims with scholarly evidence.

1. Outline common classes of medications used to manage the disease. Be sure to provide examples from scholarly evidence to support your claims.

Assess the extent to which current patient care technologies improve health promotion for your selected case, using scholarly evidence to support your claims.

Assess the extent to which current patient care technologies assist in decreasing medication errors and improving patient safety with regard to your selected case.

1. Evaluation

Identify the most appropriate measures for determining the success of your plan of care. In other words, what data will you need to collect to determine if your plan worked? Be sure to substantiate your claims with scholarly evidence.

In terms of the outcome measures you identified, evaluate the potential success of your plan of care and proposed nursing interventions for your selected case. In other words, do you feel your proposed plan of care and interventions for your selected case will result in successful outcomes? Be sure to justify your rationale with evidence-based research.

Follow the PIE format as outlined in the assignment instructions

1. Ensure your long term and short term goals are separate and unique, so essentially you are creating 4 goals alinged with the disease with two being able to met in the short term (within months) and others are long term (multiple months or years until they are expected to be met)

Ensure your goals are realistic (return to normal blood pressures for a patient with hypertension may not be realistic but a range of numbers to be within for a patient is realistic)

1. Ensure your goals are measurable (patient feels comfortable is not measurable, patient has a pain score of 3 or less on a 0-10 scale is measurable, patient is happy is not measurable, patient is able to provide examples of improved quality of life is measurable)

Ensure you research and find technologies that support your goals and are well aligned with the disease

1. When completing the intervention section ensure you include all potential medications, if the disease uses OTC medications regularly then include those in addition to prescription medications

The evaluation is what you would expect to happen, with a future focus

Ensure citations and references are included throughout, you want each of the three sections of the PIE focus to be based in evidence, not just want you think, find resources to back up each section
